Our Cancer Care Navigator program (Upstate / Downstate (PDF)) can help your clients who are diagnosed with cancer. The program can help them understand their cancer diagnosis and treatment plans, make sense of benefit options, and connect to helpful resources in their community.
We use predictive modeling to identify members who could benefit the most from proactive outreach. Cancer Care Navigators will contact the member by phone to explain how the program works and ask them to participate.
Cancer Care Navigators serve as the central point of contact at Empire for members and their cancer care teams. They work to reduce the burdens facing many members with cancer by:
• Supporting member and caregiver needs.
• Connecting the member to critical Empire and community resources.
• Reinforcing treatment plans and medications, and providing education about possible side effects.
The program is designed to improve the patient experience, improve quality outcomes, and reduce costs. Members may avoid costly emergency room visits or inpatient admissions with the navigators’ targeted, proactive support.
