We are focused on creating meaningful connections through empathetic solutions like our Concierge Care digital program. It’s available at no additional cost for clients or their employees. Members diagnosed with Crohn’s disease, complex cancer, or certain behavioral health conditions can now connect digitally to their case manager or Care Navigator through the Concierge Care app. It includes a personalized daily program for disease management. Concierge Care connects them to care in simpler, more effective, and more affordable ways. From September through the end of the year, we’ll reach out to eligible members and encourage them to download the app. Using it can help them become their own health advocate and learn positive daily habits.
Concierge Care supports members with real-time access and connects their entire care team, helping to provide the right care at the right time. The app also offers personalized content and tools curated to meet each members’ needs. It allows the care team to triage, monitor, and engage with members, and track progress over time. These abilities provide predictive insights for earlier identification of care gaps and acute care needs. The program leads to better outcomes by encouraging preventive health and real-time support in times of need, that also lowers costs for your clients and their employees.
