As we reimagine how to make our clients’ benefit administration easier, we’re simplifying how members receive Evidence of Insurability forms for most large group clients. When members need to submit information, Anthem’s Medical Evidence Underwriting department will now send the forms directly to them. Clients will no longer need to provide Evidence of Insurability forms to their employees. This change will help members access their group life coverage benefits sooner.
Medical Evidence Underwriting will send out Evidence of Insurability forms to members when we receive requests for life benefits over their plan’s guaranteed issue limit. We’ll also send out Evidence of Insurability forms to late entrants.
